Responsibilities

Conduct comprehensive assesments of client's need, strengths and risks through interviews, home visits and case evaluations.Develop, implement and review individualised care or intervention plans.Work collaboratively with government agencies, NGOs and community partners.Maintain accurate, confidential and up to date case records and reports.Monitor client progress and adjust interventions as needed.Provide support for tasks and operations as required

Requirements

Bachelor's Degree in Social Work or a related field.Strong team player with excellent communication and interpersonal skills.Passionate about working with vulnerable groups.Fluent in English and Malay; knowledge of local dialects is an added advantage.Demonstrate strong initiative and the ability to perform under pressure.Posses own transport and willing to travel for fieldwork.
